Dental Benefits

TML IEBP offers two dental benefit plan options. Both dental benefit plans cover Preventive, Basic and Major Dental services. There is no Dental network on either plan. Members can see any dentist they like, and claims are paid according to the plan's benefit schedule. Late entrants are limited to Preventive and Basic dental benefits during the first twelve (12) months of coverage. Following are details about the Dental II and Dental III benefit plans:

Dental II
The Dental II Benefit Plan is a scheduled benefit plan. Eligible Preventive, Basic and Major dental expenses are reimbursed at a fixed dollar amount based on the ADA procedure code. Basic and Major dental services are subject to a $50 calendar year deductible, and there is an annual benefit maximum of $1,200. Orthodontic procedures are not covered under the Dental II benefit plan.

Dental III
The Dental III Benefit Plan is a percentage benefit plan. The plan pays 100% for Preventive dental services, 80% for Basic dental services and 50% for Major dental services. All dental services are subject to reasonable and customary limitations. There is a $50 calendar year deductible for Basic and Major dental services, and the annual benefit maximum is $1,500.

In addition to Preventive, Basic and Major services, the Dental III plan covers Orthodontic Services. After satisfying a $50 lifetime deductible, Orthodontic services are reimbursed at 50% of reasonable and customary. Orthodontic services are limited to dependents under age 19 and are subject to a $1,500 lifetime maximum.
