Reports to: Medical Management Team Manager
Exempt, Non-Supervisory
Responsibilities:
Responsible to provide high quality telephonic care management of specified population. Be able to assess member’s readiness to change, utilizing Prochaska Model, apply appropriate intervention. Apply behavioral goal setting, education, and quality outcomes. Provide consistent review of member data for education, intervention, and re-stratification purposes. Will identify barriers to care and assist with resolutions. Provide education and behavioral intervention to assist member with achievement of goals.
Qualifications/Skills required:
Must be a Licensed Professional Counselor (LPC). Behavioral Health experience required. Computer experience; specifically Microsoft Office and medical related software programs required. Experience in telephonic disease management or case management experience with 3-5 years job specific Healthcare experience. Reports to: Customer Service Coordinator
Non-Exempt, Non-Supervisory
Responsibilities:
Completes CSCL entry of calls and referrals for adjustments if necessary Completes benefit verification and provider network participation. Provides claims status, reimbursement explanation and responds to other Member and Provider inquiries verbally and written via online requests. Refers PPO/Plan building and eligibility inquiries as necessary Communicates complex claims issues and refers escalated calls to immediate supervisor if necessary. Protects the confidentiality, integrity and availability of information and records that TML IEBP creates, receives, maintains or transmits by complying with TML IEBP policies and procedures, including policies and procedures on physical, administrative and technical safeguards.
Qualifications/Skills required:
Working knowledge of medical/dental/vision processing principles and benefit administration. Working knowledge of medical terminology. Effectively and efficiently assesses and responds to Member/Provider and group inquires, including strong closure skills. Ability to type 40 words per minute and ten-key by touch is required. Ability to provide 6.5 manned phone hours per day and log required calls per day as defined by the specific range criteria the individual occupies with minimum quality rating of 95% and or a 5 minute or less talk time average. One year customer service, billing or processing experience in a medical claims office or related setting. Must also have the ability to effectively work in a team environment and perform multi-task job functions.

Responsibilities:
Responsible for telephonic nutritional assessment, counseling and consultation. Will be responsible for evaluation of member performance improvement. Communicates member status and goals to Team Members. Responsible for data entry of counseling and consultation notes. Utilizes Prochaska Model for interviews.
Qualifications/Skills required:
Registered Dietician and CDE required. Candidate should have 3 years recent experience with 1 year of hospital experience. Experience with diabetes and Cardiovascular conditions. Excellent communication skills (verbal and written). Computer knowledge skills to include Microsoft Office and various medical computer programs.
